Yes it is open world, but it follows the pattern of other RPGs such as Gothic 3 or WoW. Yes, you can go anywhere, but if you wander into a high-level area before you're ready you'll be quickly killed.
And its free, what more can you ask for? Calling it a mod really isnt fair. Its a full-sized game thats been released for free. Seriously mate, pick a game thats been suggested and just go with it. Despite what you may think now, you may end up absolutly loving it.

I listed Gothic 3 and Beth open world rpgs as games I have already played(MW had high level areas). That is not a turn off. I will find out more about this Nerhim then.
you're going to have to be a little more flexible here. the number of recent high fantasy open world games is quite limited. If you haven't yet, pick up Fallout 3 or New Vegas. those are probably more RPG than they are shooter. I hear that Just Cause 2 had a quite exapnsive open setting for its sandbox gameplay, so it's worth a look -- possibly is more free-form than Far Cry 2. On the other hand when you play in sandboxes you have to expect finding some number of small dried turds, In this case I'll advise you to steer clear of Two Worlds, it does have a huge open world that's nicely populated and modeled, but there's little else going for it, and after playing Oblivion, the experience was truly soporific. I'm assuming that satisfying your desire for open world games requires more than some pretty vistas.
I liked TW2 but I have no intention of playing the first one. And of course I have picked up the Fallouts, I listed them in my opening post. I consider them to be deep rpgs, that happen to have fps like combat.
The reason it's not necessarily a sandbox RPG is because there are a couple areas you can't get to by adventuring, and the enemies are scaled in a way that you'll be dead very quickly if you try and explore a whole lot. After you're a decent level, you can go anywhere.
Scared 2 did not work out, MW view distance is 4 or 5 times what scared is. I can see why people don't call it open world, I hate the camera(try messing with the option.txt, still svcked) and the simple combat was not for me.
Perhaps this Nerhim will be good...
