They shout a fire/ice breath attack and then swipe/bite when they land. I've never once been hit by another shout (except for one very unique dragon) by any of them I've fought - over 30 including completion of the main quest.

I was fighting a blood dragon in Winterhold the other day - he fus-roh-dah'd me. Sent me and Lydia - along with 2 guards - flying . . . and followed that up a few minutes later by doing it again and tossing the skeleton of a dragon I'd killed earlier at me.
So, yes, they shout.
