Who do you think is going to tell Todd Howard "NO"? No one that wants to keep their job.
If the person at the top doesn't want whatever changes is needed than it won't happen.

It could be Todd's fault, or it could be a larger problem with the over company culture.
On a side note, the reason I like the idea of groupthink is that it explains their incompetence without actually making them incompetent.
