I think Milk-Drinker means that they're a wuss!

It means you are a child, because children drink milk and real men drink mead.

Vikings had similar insults; saying another viking (man) "had no beard" was a deathly serious insult (literally- people got killed over it, because it was the same as saying he was no man at all). But that only works when the target of the insult is expected to be manly. In a world where females have equal capability and respect as warriors, you'd need an insult that implies weakness and works regardless of the targets six... "milk drinker" fills the bill in Skyrim, with a decent Nord (viking) flavor.

It means your a farmer and not a warrior... It was a common european insult. Farmers tended to be able to (and still are able to) down more milk than other people because of their involvment with milk. Your ability to process milk goes down as you age, so advlts tend to not drink as much of it, except for farmers.

Mead and Wine are for grown ups, Milk is for kids. So walking into an inn and being called a "Milk-Drinker" means you are too weak to handle the grown up drinks.
