Xbox 360 Complete Bug/Glitch List

Nothing like buying a game and having it break, rendering it unplayable for days now. The stupid Xbox 360 DLC save glitch has ruined this game completely for me as all of my saves are corrupted. Personally I'm giving it another week then I'm taking it back for a refund since the product is broken.

I'm seeing a lot of bugs as well, including a few that are mentioned in the opening post (like the flailing arms in first person mode and the caravan card bug). Here are some of the bugs I encountered that aren't listed yet:

Questline Bug(s)

Quest "My Kind of Town": Completion can result in everyone being disgusted by the occupation of Primm by the NCR, even when you convinced Meyers to be the sheriff.

Quest "My Kind of Town": Meyers can turn hostile automatically at the NCRCF even when you don't accidently hit him. This shouldn't happen since he isn't a powder ganger.

Quest "My Kind of Town": Meyers can't be recruited anymore after completion of "I Fought the Law", he gets stuck in a dialogue loop.

Quest "I Fought the Law": Upon completion, Radio New Vegas always reports that the raid on the NCRCF by the NCR has failed, even when the raid was succesful.

Enemy NPC Bug(s)

A very bad spawning point just outside of Goodsprings, close to Victor's Shack, where powder gangers keep spawning out of thin air right beside you.

Visual / Audio Bug(s)

Some powder charges between the NCRCF and the Power Ganger Camp South are floating a metre above ground, at least in my game.


The devs should also check The Vault if possible, they list bugs for each quest.

Thanks for these, and for putting them in the format - added all of them - hopefully the patch will be coming soon!

I just found another game breaking bug if not mentioned before. Did the trooper andy side quest, grabbed ncr armor off dead bodies at sector charlie, traveled to mojave outpost and now everytime i access my pip boy menu, no matter where i am, game crashes. Im continuing to attempt to get through this at the moment and now, while typing this, my game crashed for 8th consecutive time. Im 13-14 hrs in and previously experienced no real problems, but now i obviously cant continue character at all. Any similar issues with constant pip boy menu causing game to freeze?

I was having the black screen of death with the entrance to the strip, but a random piece of advice on fallout wikia told to use the old cowboy hat to fix it. then suddenly everytime I tried to use computers, it would say my disc was unreadable. after seeing that 6 times, I figured maybe clearing my files and just starting fresh couldn't hurt. I was wrong. Now the game freezes when I talk to doc mitchell when he hands the mirror to you. I'm hoping a patch fixes this, because at this point i can't do a thing.

Just encountered a bug.

Went back to Novac to get the special Perk from Ranger Andy.

After talking to him it rewards me the perk. As soon it as I try to talk to him again the message appears again telling me I have received the perk, the game then freezes. (however I can still hear the radio).

Edit:

Occurred the second time I tried it too.

Third time I reentered the house after initially receiving the perk. Game gave me the message again and froze. Also noticed I can access the start menu.

Fourth time I used the start menu and loaded the auto save. This time when I talk to him the camera just zooms in and back out. I can't talk to him but the game did not freeze this time.

Fifth time I talked to him about Station Charlie first, then talked to him again to get the perk by doing the speech challenged. I had no problems whatsoever this time.

If you use the NCR radio and have another NCR person follow you around the game studders. Going into buildings with the NCR person and another companion (Boone in this case) makes the game freeze.

If you choose to kill cachino in the "How little we know" quest, it unlocks a different set of goals. I cannot blackmail the other two (Clanden and Toike) and the dead hoker does not act like she is dead even though you get the option to perform an autopsy.

Great game... hard to play due to technical difficulties... (probably would have been better as just DLC for fallout 3, just to keep the glitches to a minimum)
bug 1)
weathered pistol with laser sight... gun disappears, replaced by obnoxious red square with white explanation point on it...

bug 2)
NCR camp outside of Primm when you pick up the ncr armor from inside the storage tent it sometimes shows up as multiple stacked in your inventory, attempting to drop, wear or repair almost always freezes the game...

bug 3)
in freeside the kids that chase the rat... if you kill the rat, the kids will try to loot the rats corpse, several times i have seen the kids sliding around on their knees acting as if they are looting the rat...

bug 4)
companion achievement "the gangs all here" is definitely broken... i have met and teamed up all available companions, but the achievement stays incomplete... (also i cannot send them to the lucky 38 anymore... started out that i could... but that option then just kinda disappeared... it was the best option for the ones you didn't wanna bring with you...)

bug 5)
at vault 11 when you swim through the underwater rooms, your companion will almost always get stuck in the ceiling somehow...(there is a place you can swim up for air and you can see your companion's arm like pacing back and forth just outside of the room... as if trapped in the ceiling), and you have to completely leave the vault for them to come back...

bug 6)
if you steal the Van Graff combat armor from the chest outside silver rush and mod the plasma riffle you get to keep it... permanently, and actually cant remove it from your inventory until after completing the van graff story line when they give you the armor... which neatly stacks on the one you have, freezing the game when you try to remove, drop or repair the armor...

bug 7)
invisible walls on the mountains... (im guessing it a glitch cause besides the fact that you can roam freely in fallout 3 you don't just run into nothing and have to stop... doesn't make any sense...)

bug 8/9)
by the Helios One power station... south i think (noticed when leaving gibson junkyard to go to helios one) there is a section of rocks on the landscape that just kinda hovers... and you can walk right through it...
south of Primm when heading towards the dry lake bed... there is a floating bush on the hillside...
there are lots of other locations that this same phenomenon happens... with other types of foliage or rocks...

bug 10)
when leaving items in ed-e inventory during his run in the lorenzo from the hidden valley brotherhood of steel bunker, he is returned to primm with an empty inventory... (kinda implies they just kinda swapped him out for a flash new one... had to ask myself "wow... where's the love???")

bug 11)
when doing the missions for Diane and jack (Great Khans), if you bring a companion to vault 3... they are attacked or attack even after you have completed the speech challenge...

bug 12)
Cass's Quest "heartache by the number" does not end until you turn her over to the Vangraffs... it just stays active... even after all the steps are complete...

bug 13)
if you travel to the botherhood of steal safehouse, then head west to the Train tunnels, after picking the very hard lock on the door, there is a section inside of the tunnel with a door and a small room up some stairs... although when you try to walk up the stairs into the room you fall through the map and end up like instantly standing at either end of the tunnel, or you fall through the map a few times then the game freezes... also you can walk through the wall directly across from the small room and see... um... the matrix? really lame for a place you need 100 lockpick skill to access...

bug 14/15)
after teaming up with the guys in the old church basemant in Camp Searchlight you raid the police station... then go to raid the fire station, quest npc is almost always instantly killed by the giant scorpion waiting on the ground floor, and half of the time the scorpion is stuck in either the walls, floor, or ceiling...
also there is another glitch located in the same town... when you meet local ncr troop you asked to retrieve the dogtags of the ncr ghouls... well not all of them have dog tags... and im guessing you supposed to find 10... there are only 9.... and have have walked through every square inch of that town, the airport, and the goldmine... kinda thinking the 10th one is missing or maybe the body fell through the map or something...

bug 16)
after getting the new ed-e sometimes when traveling into the hunters farm house, ed-e has antennas and a laser barrel that just kinda float freely around him... not attached... also happens to companion's weapons(just located in the wrong spot, floating freely or passing through the character as if mortally impaled by their weapon...)

bug 17)
jumped over a road center divider(or something else about the same height) and froze my game... (happens a lot... not entirely sure if the jump caused it....)

bug 18)
when you try to bring up vats(mid combat) while reloading sometimes you target the air... like nothing appears to be targeted.... but vats acts like there is... sometimes freezes the game if you try to change your selection to a real target....

bug 19)
you can overfill your companion sometimes... not sure about the exact combination of items that causes it... but i think it has something to do with filing up the companion then adding it such small quantities of ammo that they register as less than 1 lbs..... so you can continue doing this until you have like almost everything on your companion...

bug 20)
when you do the brotherhood of steel missions (the ones from the hidden valley bunker) you are sent out to find the 2 pairs of power armor wearing paladins and the single paladin (sent to Nellis AFB), if you find them before you officially start the quest... there are normal numbers of bodies... if you find them after you start the quest you normally only find the one with the holotape... the other body just kinda isn't anywhere to be found...

20 ideas on things to add to the fix list... kinda makes the game really hard to want to play... SO... let's get on that making and releasing that patch please! GOODLUCK!

Which shelf would that be? I've been all over the Hidden Supply Cave and have only gotten the one rad suit, helmet, gargnak the barbarian, and the med supplies.


There's a metal shelf like you see all over the wastes and in vaults. It should be on the middle shelf. Not sure if it's always there or you need to activate it by getting the intel from boxcars.

Here is another bug...

After you do Veronica's quest line, you are ambushed by BoS upon leaving the bunker. If you cannot pass the speech challenge which is 95, you fight them. After this moment, veronica will not follow you anymore even though she says she will. You can send her to your house or back to the trade post. You can move her from location to location if you make her unconcious then fast travel, but she will not move in the new area either. So basically, get 95 speech before you finish her quest line or she will become useless.

The bug has something to do with her trying to talk to you all the time. Every time you knock her unconcious and fast travel, dialogue will open up as if you clicked her to talk.

Also to be added to "E-DE my love" if you talk to the old scrap yard grandma before you have e-de she tells you all about E-DE but then if you get the quest after she won't repeat herself

you only get 3 options, Goodbye, "I'd like to repair" and "i'd like to sell." so quest is broke for me.

and this is not a glitch but it makes me sad you can't get Tabitha's wig or sunglasses D:

I've only been playing the game for about 4 hours and have already encountered a few bugs. Fortunately nothing too serious.

I guess problems that people have already encountered:

> My console froze just outside Goodsprings on the road to Jean's Sky Diving hut.
> A coyote randomly spawned by the campfire near Goodsprings Source when I was about to talk to Sunny Smiles.
> A bloatfly by Jean's Sky Diving was floating inside the scenery.
> A small radscorpion was inside the scenery on the road from the Mojave Outpost to Nipton (by the small drive-in but I can't remember the name of the place).
> My arms flicker up and down the screen on occasions. Putting my weapon away fixes the issue.
> I've also encountered a few graphical glitches

I haven't seen this one reported yet:

> In VATS targets may appear partially off the top of the screen making it difficult to target individual body parts and see the hit stats for that body part. I believe exiting VATS and then going back into it fixes the issue, but I can't remember as I was in the middle of a frantic firefight at the time.

For information, I am running a refurbed console without an HDMI socket. The game isn't installed on my hard drive and I have the patch released last Friday. I started playing the game after the patch was released.

It's a real shame there are so many issues with this game as I think once they've been resolved it will be truly awesome. I'm enjoying it - but that enjoyment is spoilt to an extent by the constant worry of when the next glitch will occur.

I feel there is a real lack of meaningful information coming from Bethesda/Obsidian on patches. Now they've got our hard earned money they've run away into the sunset. An update on future patches/progress fixing the bugs would be nice.

I just got "the gangs all here" (ending with Raul) so didn't experience that. But as for the companions not having the option to send them back to the lucky 38. It's the patch that did it. Don't know why, and that's one of the reasons I play with out the patch. Besides the fact that the patch doesn't seem to have fixed any problems.

I have had a similar issue, but with the ED-D my love quest line. But the result is the same as yours. Can't move or open menus after dialogue ends. :(

Wow.. 11 pages of bugs. I skimmed through, and I didn't see this mentioned, but it may have been. Nevertheless, it's worth mentioning again because it may save someone a headache.

Killing Caleb McCaffery on The Strip will result in constant freezing every time you try to re-enter the strip (obviously rendering the game useless). I'm sure not all will experience it, but many other forums I've visited, I read others complaining of the same thing. Lucky for me I have saved game prior to killing him (although quite a ways back) otherwise it would have meant starting all over.
The reported solution is to fire a few shots at him, luring him to follow you out to Freeside and kill him there, or to complete the mission through speech. I haven't got myself back to that point yet to try it.

FTR: installing to hard drive, current patch, clearing cache, dismissing companions, etc etc all had no results in helping this.

Hope this helps someone.
Love the game, but some of this stuff REALLY needs to be sorted out.

****************Update on thies issue***************

I tried the apparent solution of luring McCaffery to Freeside before offing him... all it did was move the freeze issue to Freeside. lol. Seems as though wherever his body lies, you'll no longer have access.
The only solution I have found is to NOT kill him at all. ...instead, convince him to give you his hat (providing your speech skill is high enough). Seems to be ok, after doing that.

i tried adding the laser sight to my weathered 10mm. now i have no gun in my hand when equiped and a red and white translucent box around the upper half of my body, but it still works

*** this **** i need this fixed asap. makes me so mad. Since i cant complete the brotherhood quests because the elder and hardin are stuck in speech loops, veronica's the only one who can wear power armor. i think. i need her as a companion.

My apologoes if this has already been noted

BoS base L1 - Medical room - Medical case on wall cannot be clicked on to open

BoS base L2 - repair room - Damged armour/weapons cannot be picked up even though they are highlighted with "A: steal damaged laser rifle", clicking on them seems to still lower your karma though (not 100% sure on that one)

BoS 1st mission to remove the NCR ranger, if you tell the ranger about the Bos base you are attacked. After defeating the BoS I tried to give EDe and Boone the power armour and it come up with "... cannot wear this as it is not of their faction" and they drop the armour (dont even carry it), Boone is able to wear the helmet though (must be power armour trained). However when in the base I gave boone the power armour from the armoury and he was able to wear it with no issues. Boones carrying capacity doesnt increase with his added strangth from the armour either.

REPCONN Headquarters L1 - shop store room - there are 2 safes overlaid on top of one another, after picking one safe and looting it if you move your crosshairs over the safe again it will change from open safe to picklock and back and forth so you can pick the safe twice for twice the reward (minor reward but still a fault)

"The house always wins" quest fails if you finish the Vault 11 story early in the game
and
Euclid's C-Finder sometimes will never get a charge if you get the gun after completing the quest (even if waiting a few days)
had both of them happen to me and to some people on gamefaqs

It looks like it's activated by talking to Boxcars, I checked the one you mean and it's empty. That svcks, kind of takes away the do anything anytime feel of the game.

I just managed to get Arcade Gannon as a companion along with having Boone and Rex, which I though I couldn't do (and also tried previously). I can open all their companion wheels and they're all following me. Is this a bug, or is it something thatyou can actually do in the game (have 2 people + 1 non-person following)?

Oh, also, another bug - The game told me Cass had returned to the Mojave Outpost and that I had lost her perk. I'd already killed her earlier in the game.

Ive read through and not seen this mentioned yet.

Anytime I enter a building in which I've killed things, upon re-entering the building the game freezes for a few seconds and then the bodies of everything I previously killed inside will appear in front of my character in mid air and fall to the ground. This is accompanied by the sound of tons of metal scraping/falling extremely loud. All the bodies will remain there twitching around at the entrance to the sound of tons of metal scraping /falling. If I exit the building and re-enter the same thing happens. First time it happened, the insane crashing sound scared the life out of me and almost blew my speakers. The pile of bodies that materialized and crashed to the floor actually made me chuckle. Then I discovered to my dismay that its not going away. Loading previous saves does not change it.

It's gotten to the point that I play with no sound.

There's an invisible fence on rocks just east of, and very close to, the Coyote Mines house. I can climb to the top of the rocks, but can't walk down the other side
