Ah, sorry. I know of the existence of and have played the first two games, I somehow missed the "Fallout games have pushed the envelope" point that started the whole thing. Entirely my fault, I apologize. I've been thinking in terms of Bethesda's Fallout series, I barely connect them at times.
Allow me to redeem myself with something that actually makes sense in context.
The industry has changed. During the release of Fallout 1 and Fallout 2, gaming was more of a niche market. Especially when it comes to computer RPGs, rather than console platformers. Cost and risk anolysis change when you aren't developing for a niche market.
Bethesda pours millions of dollars into these games. These games have to sell to a much wider audience to be considered a success. I'm pretty sure it was stated that Fallout 3 has since sold far more copies than 1 and 2 ever did combined. Gaming is a new beast, at least the market Bethesda is aiming for. Mass market appeal, means mass market exposure....and mass controversy, which could throw a wrench in the expected earnings of a millions of dollars project.
And remember, child killing WAS an issue out there, beyond the sea, somewhere.
