For me it would be
1: The 'hardcoe' mode, eating, drinking, sleeping etc
2: More tactical combat. I don't mean overly complex and clunky (like The Witcher 2), but armour gives less protection against serious blows, requiring even high level tanks to retreat sometimes
3: More spells/spell maing

How was combat in The Witcher 2 complex? You could dodge with rolling, Block or counter attack. Magic was incredibly diluted. As far as games go, The Witcher 2's combat system was bare bones.
